Turn-based mode (not in combat?) skips turns automatically.

My companions are dead or almost dead. They are not grouped with me. I enter turn-based mode just as a fight ends. (I later notice a hostile tadpole nearby, but we are not engaged in a fight.)
But, every time the 6 second period ends, my turn is skipped automatically. I only have a very short moment to do anything. It's as if turn-based is happening in real time.
I can't exit turn-based mode (it's Shift+Space, right?), likely because I can't time it for my turns.

What did I do?

I'm leaving a slightly different issue here, tho the subject line is pretty much the same so figure I won't make a whole new post.

Sometimes when my chars initiative is "tied", represented by intertwined portraits at top left in combat UI, the system will end turn for each of them after the first one goes. So if Gale and Shadowheart are bound up, I sometimes have Gale do a thing, end his turn, and Shadowheart gets skipped.

This *may* happen (more?) when I click in to Shadowheart before ending Gale's turn, but I'm not positive.

I also experienced this enough times that I started getting into the habit of clicking on everyone's linked portrait to take their turn before ending anyone's turns. I don't know what causes someone's turn to get completely skipped like that, but the round will just end without coming back to them.

As to the OP's question, I don't have any familiarity with using turn-based mode outside of combat, but this sounds like a bug.

It just happened to me tonight. When I entered the fight, I was placed right behind one of my companions, who was already in the fight, and my turn was skipped entirely even though I wasn't surprised.
