I first noticed this for the scroll of Resistance. The actual scroll icon has the correct image I assume, however the tool tip or expaned icon, whatever you want to call it when you hover over the item to read what it is, shows the image for the scroll of ray of sickness instead. This is the first I've seen with this and none of my other items do this currently, but I'll add any other examples of this below.
