Occurred for the first time yesterday during the Phase Spider fight, then again in the Zhentarim hideout. Battle is initiated, but all enemies merely "plot their next move," typically for an excessive amount of time (10 - 15 secs), but don't move and don't attack. Needless to say they were all slaughtered, but that's unsatisfying of course. Anyone else encounter this issue?

Yep - yesterday I had this exact thing happen with some mud mephits that were summoned from some other mud mephits. Like your experience, they just plotted their next move and did nothing. I even reloaded from a save before the combat and it happened again.

I would say it happened in 50% of the fights I encountered in the game. And it's frustrating indeed.

i believe it's some kind of pathing problem.

For instance the fight with the ancient mudlings went like this:

the ancient mudlings summoned the younglings, who just stood there.
the first wood treant went through the entangling thorns towards my party on the small island, the second one had his path blocked by the first one, so he went for the closest edge of the docks and just stood there for the rest of the fight.

The fight with the goblin camp went like this:

i was standing on top, where the drunk goblins slept (Allready killed them before the fight with the whole camp).
every ranged goblin was attacking me like normal and the melee ones just stood there, as if they did not know that there was a path towards my group ( a very very long path).
they only started moving when astarion got knocked down and was a viable attack option in range.

For me, it started with my player character casting a scroll of Witch Bolt on the Phase Spider Matriarch. The bolt tethered me to the target, as expected. That's when the "plotting next move" business began. Three spiderlings and the matriarch, effectively all just sitting there. No one blocked anyone's path, in this case (which would be difficult to do with phasing creatures, anyway, right?).

The weird thing is that once this happens in one fight, it apparently carries over to all subsequent encounters, and persists both after reloads and complete restarts of the game. It's like a cheat button you can't turn off. Certainly something that needs to be fixed.

I forgot to mention... the other thing that started occurring is that regardless where I am when I save a game, on reload I am now back where the Phase Spider fight took place, although the state of all locations, characters, and my inventory will reflect what happened before I saved the game. My party, on the other hand, will be in the location where I saved the game. Really weird. I've no idea if this is connected to the unresponsive enemies, however.
