Lae'zel's Riposte ability disappeared

I recently hit level 4 on one of my playthroughs. At level 2 or 3, can't remember which, I took the Riposte ability for Lae'zel. I noticed it is gone from her reaction box since I hit level 4. Any suggestions? That was a devastatingly effective ability.

A fight didn't go my way and I reloaded a previous save and her Riposte ability is back! Not sure why it happened or how to duplicate the bug but I'm glad it fixed itself.

Also lost the riposte ability when leveling to 4th. Disappeared from the reaction box and missing from the skills list.
