It's worth noting that in this particular save I have two dead followers attached to my CaC from the Underdark that had died but never left the icon after I completed the Sovereign mission ended and he left my party. No real issue there I noticed except the icons on screen but who knows. On to the crash...
When I tried throwing Alchemist's Fire on the four guards trying to force open the door, the game will crash right after initiative is rolled. This happened no matter how variable I tried to get in changing the circumstances of throwing the Alchemist's fire on the Flaming Fist members trying to force the door. Going into tactical mode allowed the game to last a little longer but it still crashes before the first turn in combat can be completed. Attacking the same group more conventionally allowed the fight to play.
Tangential note- you can't kill the lady you're meant to rescue(was going for a pure "evil" playthrough) though you can put her HP to 0.
