Regarding the Hag in the Tea-house
The hag spawned without talking and is still described as Level 1 Auntie Ethel. The model of a hag is used..

1. I sneaked past the hag, down into her basement and look at her cauldron. I'm alone with her trophies.
2. I look at the mirror.
3. The hag appears next to her cauldron and does nothing. She just stands there.
4. The description says "Level 1 Auntie Ethel" with 112 HP. But it's the model of the hag.
4. Can't talk to her. I could still attack her.

Conjecture
- Ethel should want to talk to me or attack me. After all, she does attack, if I get cought sneaking past her.
- Ethel should not immediately look like a hag.

Beforehand: I declined her offer when talking up in the hut.

I kill the hag, but no one acknowledges it.

The door doesn't react, despite the hags carcass being right there.
Neither does Lorin.

EDIT:
Just had a talk with the door as Gale, after my main. Gale gets the text *You feel the barest hint of life from the door* and thats it. He can't say anything. He can't attack. Nothing reacts.
Switching to other characters works, but they can't do anything to Gale. Except killing him.

EDIT2:
Killing Gale worked.

Spontaneous reappearance of the hag.

I descend further into the cellar. After the area with the fumes comes a cut-scene where I should get to free the girl.
The hag is in the cut-scene well and healthy. The hag starts combat.

But this time it shows the hag as:
"Auntie Ethel
Sister of the seeing Pearl
112/112
Level 5"

I think the game handles the Auntie Ethel you talk to and the one you are supposed to fight as two different characters. Which would explain why I can fight and kill her twice.

I killed her in her hut after she went invisible. Put some poison on the ground where she was and she reappeared. I think she was trying to make it to her fireplace, but I killed her before she could. I dont recall her really do much in the way of dmg at all doing it this way.
