I've had this issue across two different multiplayer games; my companion (in the first case Lae'zel, in the second case Shadowheart) approaches an enemy, then when positioned in melee, I use the attack action to hit the enemy. My "action" resource is used up, but no attack animation is played by the companion, and no attack roll or damage are calculated towards the enemy. Instead, the enemy enters their attack animation and they are given an attack against my companion for seemingly no reason. Either it's calculating an attack as something which can trigger an attack of opportunity, or it's just straight-up glitching out and causing my companion's attack action to be initiated by the enemy against my companion rather than the other way around. It doesn't happen often, I think once or twice for each of them in two radically different fights across maybe 10 hours of gameplay for each of the two playthroughs, but it's a very obvious and quite infuriating bug when it does crop up.
I've got a few other gripes, but this is definitely the biggest one so far. Overall I've highly enjoyed the game and can't wait for more content to be released.
