I have had this same glitch happen twice, with two different characters. Both times Shadowheart was *not* in my party (I had recruited her, she was just left at camp) and I returned to camp to rest triggering the cinematic with Raphael. Once the cinematic is through and you are returned to camp, Shadowheart disappears and is nowhere to be found, in or outside of camp. Reloading from a prior save didn't do anything, she was still gone after the cinematic ended. If she is in the party, this hasn't happened, and no other characters disappear afterward.
