I've had this happen twice.
Once when being beaten by that priest in the goblin camp that wants you to take hits.
Now the zhent camp. As mentioned, I picked up a oil barrel by accident. Got attacked.
Reloaded. And save before zhent conversation, checking the quest log, shows updated that returning the missing shipment failed.
