She's right there in camp in a puddle of her own blood, totally dead.
Except I can't revivify her, the Talkative Skeleton doesn't register that she's dead, (so, cant resurrect her) and she's still approving/disapproving actions.
I suspect Lae'zel. If it had been Astarion, he wouldn't lave let all that blood go to waste.

This happened to me once too, but it was on the morning after I had allowed Astarion to sample my blood and then failed to persuade him to stop. I just assumed that after I passed out, his bloodlust drove him to drain Shadowheart next in order to sate his compulsion?
Either way, she was dead, kaput, gone for good in that playthrough for me as well, no revival possible.

This also happened to me after failing the persuasion check during the Astation feeding cutscene! (I had to reload). Curiously enough, in a different playthrough, it was my PC that ended up dead after Astarion got overzealous with feeding. The game did register the PC as dead this time around, so I brought them back via Talkative Skeleton. I suspect that the second scenario (the PC dying, not Shadowheart) is how this checkpoint failure is *supposed* to go.

Weird. I don't recall failing that roll though. And this happened long after that scene.

Yeah, if they die while hostile in camp you're screwed. If you have other characters outside of the camp when that happens, I think it's better to let the hostile companion win. If they're hostile to your main, a skeleton resurrection is far enough away that they won't immediately become hostile, and a nights rest(triggered by a character they aren't hostile to) usually fixes it. If they're hostile to someone besides your main you can have them join your party, and they won't be hostile as long as they are grouped. Just don't send them back to camp while the character they are hostile to is in combat range.

Something similar just happened to me... I was fighting the hag and Gale fell to his death. I keep fighting and a few turns later I get a journal update kindly warning me that "Astarion is dead. No time for a funeral." What!? Then it hit me... Gale's corpse. Of course. Damn mage and his necrotic energy. Shadowheart, Astarion and Scratch (!) need protection against this walking bomb. Can we move his tent away from the others please? Or not... since this really cracked me up.  It would be funny, if companions could actually be resurrected after Gale kills them... Well, I gotta go kill that hag again I guess... thank you Gale.
