I'm no expert in design, I started to focus more on rpg games this year thanks to larian's awesome games and what I think is maybe because the game went on a cinematic route the world design got affected being too realistic. I'm not saying in a way that this is the best graphics(not that it is not appealing), what I'm saying here is like natural rocks, walls, uneven surfaces, hills etc., made me think if the world design is to blame for all the AI failing to function, ragdolls glitching and camera can't settle on a good angle most of the times. I just feel like, although cinematic look and realistic world map is a very tough job and I love it, but it feels it's not made to function with gameplay like I played divinity 2 aswell and I remember it had very even surfaces and squarish rooms to compliment gameplay and the gameplay was butter smooth.
