I was in the groove and Sazza had just been killed. I went to the secret stone door that leads to the underground passage and as soon as I crossed the door Astarion triggered the conversation with Karlach, the wounded tiefling, even though we weren't with her. As the conversation finished, Astarion was in front of the stone door again while the rest of the party were in the undergound passage.
I'm playing with the v4 and the #7 hotfix
