In the owlbear cave there is what might be falling rain near the entrance - it seems to fall in the same pattern as the rain outside but darker colour. several (single player) crew have been injured just inside the cave possibly congruent with this dark rain. Not every time and also not every time from the same save. Noticed on 4.1.99.8615 may have happened earlier
In the Tollhouse cellar when reaching for a chest behind the ladder sometimes the ladder breaks. Today Wyll was also injured for 12 HP when walking past him in the same area. I've noticed the ladder issue before 8615 I'm not sure about the walking by.
