This has not been a major issue (except for the one time I could not initiate the party scene), but from time to time a key NPC dies and you are not able to progress on important story points. For example, though it may be fixed by now, Zevlor died in a fight with the druids, so when I was ready to camp, I could not trigger the party scene. Another tiefling had taken his place as the leader, however that NPC did not have the available dialogue to trigger the party. We tried to camp and trigger it that way, but it still would not work. Another example is if Aradin dies, his surviving companions will not give you the nightsong quest.
I like that NPCs can die and change the story based on who does and does not survive. However, there are certain ones where another should take their place. I would not want to see all the dialogue options transferred from the dead NPC to the living one, but I would like to see whoever takes over be able to offer a quest, or trigger the party scene. This way, players don't have to miss out on something important because of the wrong NPC dying, or worse, not even know there was a quest available because the NPC died before a conversation could be triggered.
