You will not have to start over. I started the game under v1.29, installed the 1.32 hotfix after breaking Finnigan's magic lockpicks, installed the 1.33 patch when it came out (never encountered the black map problem) and completed the game under v1.34.
The suggestion to backup your saves before installing the patch was just in case there was a problem, but that is unlikely.
