I experienced the following issue in the German version (1.00) of the game:
Act 1, Water Quest (Get water for prisoner):
Short: Quest appears in quest log after taking the right bottle, not after talking to the prisoner.
Long:
I talked to the prisoner and told him that I'll try to find some water for him. Nothing appeared in my quest log. I went to the kitchen nevertheless and took some water bottles and cups. Unfortunately I missed the right one in the first attempt so nothing happened with the prisoner after talking to him.
I tried again and took the right bottle from the table. The quest was added to my quest log in the same second and I was able to finish it by talking to the prisoner again.
IMHO the quest should be added to the quest log after talking to the prisoner and agreeing to get the water.
