You need the alchemy skill level 1, which will enable you to mix existing potions with each other to get new, different ones.
Alchemy levels 2 - 5 will allow you to refine varying degrees of potions from herbs or mushrooms (you need empty bottles in addition) drag the bottle (size according to skill level) over the plants
