sometimes they are tucked into hallways and it's hard to see them on the map. since they are randomly generated there's no way to say where it is, but go to your map page and look for a little shot of blue or white sticking up over a wall. also make sure you have explored the whole of the level. you may have to backtrack down every hall to find it.
