your skill lvl is strange - if you didn't use one. You should have 10 IMO at lvl 9. As every 5th lvl gives 2.

There is another survival potion in Act 3 - but with a holy water bowl+ alchemy skill extract at lvl 4 + plants you can create perm. potions yourself (one for each bowl). Please, take OFF all your stuff and look if survival goes up again - maybe you have a restricted item equipped? I recall, I once had this: drank the perm survival and nothing went up - but I forgot to check in that save, if my items were the cause or if it's a bug. I know from others, they had probs in that also.
The big map => ingame? should remain uncovered - big map in BF? They reveal as you walk. Some have probs with this covering up each time they re-enter the BF.

In-game there's only one teacher in Act 1 for lvl 5 (weapon officer) and Act 3 (smith)
