All with the 1.45 patch installed:

When I was sent to Asmodeus in Act III to answer the quiz, both teleporter pyramids disappeared from my inventory (I.E. I cannot get into the lava circle with the chest).

I have fixed the teleporters but the trapped Raanaar still say I have not completed the quest (orphaned quest).

The Cursed Man takes the concotion from Raven, and it cures him but the quest is never marked as completed and the item is still in my inventory (orphaned quest).

Many doors in Zandalor's dungeon are not marked properly (object #xxxx).

The teleporter stones get replaced on act change with another set, and should end up in the hero's inventory. Were you perhaps completely loaded with loot, and couldn't take another 10 weight (assuming the DK was carrying them in act 2)?
In any case, you can replace them (Inventory files), though they disappear entering act 4, so it doesn't sound like you would have them for long.

Did you talk to all of the Raanaar? There is a specific one you need to talk to to complete the quest (they should then walk away in a cut scene).

I wasnt loaded and went back to an earlier savegame to make sure. I actually had one pyramid on the DK and one on the Hero...

Oh I almost forgot, If you combine the ingredients to make the gunpowder in Act 1 and you are not in the forge the ingredients disappear.

I have spoken with all the Ranaar and they just keep saying there must be a way to fix the teleporter etc. (even though I can use it). I believe its a bug. Just reporting this stuff so someone checks it for a possible future patch. Thanks.

Strange... I had no problems splitting the teleporters up between characters, giving both to either character or leaving one or both behind.

Some people have also had problems combining the bomb ingredients in the DK's inventory, though the mixture should just be put in the hero's inventory.

I asked about the Raanaar because when I did that quest the one I needed to talk to wandered a bit away from the others, and I didn't see him at first. If none of them acknowledge the fixed teleporter, that is a bug.
