thanks for the reply. Seems like the secondary skills icons "drop teleporter and use" and "use teleporter" do not always work. I got away from the lava problem by putting the teleporter in my inventory, then clicking on it there.
I finished the 6 dungeons of act 2, sort of. Could not find a way down to a second level in two of the six! In the last dungeon, either the 5th or the 6th, I found 3 of the quest items on the 3rd level, no bosses or monsters around, but I am giving up on solving the other quests. Could be that there is an entrance to the lower levels hidden somewhere, as you say, but I just don't want to spend so much time looking for them.
