Do you have your caps lock key enabled? Caps lock (or shift) lets you move the screen focus away from your character (to see or throw stuff farther), but can also result in the focus not following your character when using stairs or ladders.
BTW, if you hadn't noticed, there is a
patch. This may not be required for the re-release version; the version number is listed in the bottom right corner on the main menu (game version 1.0034A corrisponds to the latest 1.34 patch).
