It really doesn't matter whether you "pass" all the tests or not, although it is possible to do so. You can still continue with the storyline just fine! See
Raze's comments here. Sorry to say that the teleporter stones do indeed disappear going into Act 4.... We suppose that the imp historian came back for them!
