I solved the puzzle and found Patrick's deed. I wanted to come back for it and the items in the chest at a later time. I was going to leave when I had troubles finding the portal stone.

So I got frustrated and screwed around with all the levers. I finally noticed the portal stone and went through it. Now I come back to get the deed along with all the other [nocando], and I can't open the door. I've put the levers into their correct positions and the door won't unlock anymore.

I'm thinking I might have to figure out how to use the hex editor to get inside, but if there is any other ideas I'd love to hear them.

I also ran into another glitch, but the item I lost was crappy: my weight limit was over to the point where I could no longer carry anymore. I removed an amulet so I could replace it with one which had a +1 trader's tongue bonus. As soon as I removed the amulet and went to swap with another, TOO HEAVY TO LIFT flashed up what must've been a hundred times and then the amulet vanished.

If you put all the levers in the up position, leave using the portal stone (maybe even leave Verdistis), then return to pull the north west lever, then the south east lever, and finally the central lever, does that get the door open? The north east and south west levers should remain up.

Have you tried just playing with the levers again, to get all 4 doors open to the north and south? Once they are all open, pulling the central lever should close them and open the final door to the western room.

There can sometimes be a gap in the north west corner when two walls meet, which it may be possible to toss a teleporter stone through. You could check that corner of the treasure room in Sir Patrick's house, and see if you can see into the house from the outside, and drag and drop a teleporter in.

You could use the file swap procedure described in the FIX for the Missing Catacombs Portal topic to get into the locked room, but I would have to find one of my old saves at that point to get a suitable data file (if you have any saves from before you first entered via the portal stone, you could load that save temporarily, get into the treasure room and then save to create the necessary file to use with your latest save).

The problem with items possibly being destroyed if you are overloaded is a known issue.

Tried this and it worked. I thought about trying to reset the puzzle, only I couldn't remember how it was originally. Thanks.
