In the save you sent me I have no problem getting the teleporter stone. When you mentioned replacing the inv.b0 file before, was that from another save after getting the teleporter stones, or maybe from a save in act 1 where they might not be listed?
Anyway, the inv.b0 file from my save corrupts most of your equipment, so if the file you used does not, it might be better to use the inventory swap procedure to get the teleporter stone. Hopefully that would also trigger the game to add the icons to the secondary skill menu, though they are not really necessary.
Did you get experience points for the BF quests and they remain open in the quest log, or did you do what was necessary to complete the quest and the merchants did not recognize that? There were some problems with BF quests before the patch, but they should be mostly cleared up.
