You would have gotten that the first time though the cave (otherwise you can not get out).

Do the file swap, load the game, move one teleporter stone in the inventory, then click on it (without touching the other). If that doesn't work, re-load the save and try again with the other teleporter.
I didn't try in BD, but in DD the game didn't have a problem with two sets of teleporter stones. The last moved teleporter of each type would become the active one. If you load the save and only move one teleporter, then hopefully the game will leave the one in the mushroom forest active so you can teleport to it.
Alternately, do the file swap before triggering the trap, and after restoring your original inventory files pick up one teleporter and leave the other on the ground.
The game will show one teleporter on the ground in the little mushroom forest, but if you mouse over it before triggering the trap it will disappear (so it may not actually be there). If both teleporters are suppose to start off in the imp historian's inventory, then you will need to do the swap before the trap, or hex teleport out.
