I remember from snippets that the character can be either male or female - and so does the dragon (I think it's natural, isn't it
).
Also I
think I can remember something about dragon armor and human armor ... Although I'm not fully sure ...
3 - I don't think it can be altered totally.
5 - Only the human form, I believe, since the dragon form is part of the plot, actualkly.
1 - You must become a human in order to walk around, that's how I understood it. On the other side, a human cannot fly.
