The one on the lake with the scary ghoul noises can only be opened from the inside and to get in there you have to go all the way to the top of the tower with the ghoul smith, push the button inside the fire pit (and all 4), grab the item that spawns and double click it in quest items. Looking closer will port you in and be prepared to fight.
