1. This can be related to hardware or an old driver. We don't have any system spec. so cannot help you any more.
2. This never happened to me (or anyone else) can you go into more details? Are you saving in town and ending up loading the game but in a dungeon?
3. This sounds like an installation problem. I'd try to uninstall and reinstall the game.
