Try running the game in Win98/ME compatibility mode (I'd do that anyway to avoid a potential bug some people encountered in Vista, as mentioned above). Also in the compatibility options for Vista/7, try disabling desktop composition (IIRC that helped a couple people with stuttering problems in D2:ED, which uses a completely different engine, but it's worth trying).
