Do you mean you "see" the enemies "their HP bar is half filled" , just even before you start a fight or have ever met these enemies before ?
I play nightmare and all goes well so far (L19).
On 7th of february 2015 : I start a new adventure in the Divinity world of Original Sin, it's a Fantastic Freaking Fabulous Funny ... it's my All Time Favorite One !
