You can hold the left trigger down to lock on to a specific target. In D2:ED this also moved the camera to an almost over the shoulder view; in DKS the default camera position is closer, and unaffected by using target lock.
A voiced main character would have tripled (at least) the time and resources needed for voice acting. The amount of dialog already was also the reason there were no differences between male and female main characters.
DKS didn't add any quests to the D2:ED section of the game, though some quest rewards were changed (experience points were balanced so it was easier to level up at the start of the game, etc). Unfortunately the choices in quests didn't affect anything outside of that particular quest (or series of quests). Hopefully for their next games Larian will be able to add a little more depth.
