I heard some quests can reward Malachite Gems, so I have questions.
1: Do the quests that reward the gems ALWAYS reward them, or is it random?
2: Are the gems one of the selectable rewards, or are they "secretly" added to your inventory, like with The Horror of High Hall (in ED)?
3: Which quests can (or do) reward gems?

I've never got them as quest award in FoV, so 1 is most probably random.

1> yes, they are random. i got some in different quests as a reward- but only once, as a set of 4. 2> you select it as one of your rewards, so they don't "mysteriously" appear.

Probably all of them, they can just appear as "gem" drop...

Maybe, but I've only seen them once, as reward for Mind over Matter. Been reloading a save from before finishing to see if they pop up again, and no luck. I'm wondering if it's not just a straight 1/17 chance each quest.

In Flames of Vengeance, malachite gems were added to the list of possible quest rewards for certain big quests, i.e. main quests and some other, selected quests. So if you're lucky, they'll pop up, or you can reload and try again when – for instance – closing the quest Nericon (the talking tree) gave you.
I am in blood Stepp'd in so far, that, should I wade no more, Returning were as tedious as go o'er.

I'm assuming the "selected other quests" involve ones that include an optional rewards slot with a stack of 4 or 5 gems.
As for the quests that reward the gems, it definitely seems like a straight up 1/17 chance of Malachite Gems being chosen. I reloaded Mind over Matter about 50 times and only once did it have Malachite Gems as a reward.

Many quests have gems as an optional reward, but not all of those have malachite among the gemstone candidates. Yeah, but most quests I've completed so far have 1 or 2 gems available, and despite a lot of reloading, never saw Malachite. Mind over Matter had a stack of 5 gems in one of its slots, and it turned up Malachite twice. Since reloading to finish that quest multiple times also gives the lesser reward for just Valeri's segment of it (including a slot with 1 gem), and I never saw Malachite in it, I'm guessing only quests with stacks of gems greater than 2 have Malachite randomly.

Looks like I was wrong.
But yes, they are very rare. As said; I never got it once during my regular FoV play (then again, I didn't save-reload to try getting better). It sounds logical if the chance is 1/17 on selected quests...

That's ED though... So, what? Well, I need to add it to my guide, for one. The fact an ED quest has Malachite Gems as a possible reward (as in, an actual random gem reward) is quite unique.

Just now, yes. It's sort of a pain because you have to enter the Hall of Echoes, then load the save in the Fjords, complete with ridiculous loading time (at least on the 360 version). But a Malachite Gem indeed appears there. Only one, though, whereas quests in FoV that can give the gems give at least two at a time.

when you walk down from great market to lanlinor lane right after the 5 runes that are on the wall you wil see a statue that will ask for a password (the order to solve it is to activate 5 handles throughout Aleroth) then you have acces to the sewers if you go in there and make a left turn you'll see a melachite vein;)

I have that listed in my guide. It's FoV vein #3.
