I'd like to see both an expansion and a sequel, but if I had to choose, then, well, a sequel would be my choice, for the most important [imho] aforesaid reason, that the engine is good, so fixes here and there don't really do much, but an altogether further development would probably be something extremely good. I can't help relating this to when we [gamers] were talking about sequels to baldur's gate and the infinity engine and how it developed in icewind dale vs baldurs gate 2. But one thing I do know, that I'd like the sequel to have some form of continuation, like some solid reference to the character and events in dd1, not like in diablo2 for example, where it just says, oh the last hero got consumed by the bad guy, now you're the new guy, hello goodbye. Multi I'd like to see, but I can visualise why it would be a major bummer with dd. It's made to be single from the scratch, it seems, and even though you might even stretch it as far as having permanent companions ingame from the npc list, it would be rather unbalancing to be multi. Or else it ends up like battlenet, ok, i got this dagger, you gimme this helm, nice to meet you, let's go kill the cows now and get some items. Personally I find it boring and uneventful. Btw, my favourite thing about the game is [laugh all you want] the trophy screen. It's really something, and I love that. No slots inventory too is a HUGE pro.
