oh... Not sure what will trigger it. Try talking to the guards at the gate of the quarantied area.
Also the healers in the inn mention they're working together with the doc. But I think it's the guards that trigger it. I don't remember exactly, but I think there's even a screen where you see the doc. leave when you talk to the guards.
You don't need (aren't supposed to) lockpick the first time.
The second time you can use the key.
