If you don't click on the waypoint, it doesn't register that you've even discovered it, even though you might have gotten the exploration bonus for it. I've had this problem too, but for a different thing. I was using the waypoint at the standing stones and found out that I hadn't actually 'activated' the one in cyseal. So, long boring trek back to town
