It looks like Evelyn can break out of the first conversation properly if it is at the main level, but if I tried the same thing after asking about one of the patients, Scarlett gets stuck as you described. I found a way to free her; have Roderick sneak and get caught trying to pickpocket Evelyn (that leaves Roderick stuck, though).
