What your experiancing is a targeting issue where other items and even the main characters get in the way of your target and your mouse pointer cannot properly highlight the enemy. Sometimes rotating the camera helps. It is easy to run into this issue when combat gets intense and I've found myself cursing my characters for running around instead of attacking like expected. I'm really hoping this targeting issue is resolved prior to the release of the full game...or these forums will flood with complaints.
Everyone here in the beta has experienced it and knows your pain.
