The ALT key highlight is based on your characters perception, so it's entirely possible the fire/lava/smoke/etc could prevent you from "seeing" something.
I know, that's why I think it's stupid. I mean realistically I can understand, but trying to find that sweet spot to be able to see it with the alt key is stupid.
