Without fail during battle, the sound effects for spells, attacks, or abilities will not trigger half the time. This results in silent spells and abilities and no sound when a weapon strikes an enemy. Sometimes the sound for these effects will play and work fine, and I can't really determine a pattern for when they will and will not work.

There are also instances of it happening outside of battle in regards to the main character dialogue and that of other NPCs. Sometimes their quips will not be voiced and yet upon reloading a save and going through the same area, the sound will work as intended and I will hear them.

Any idea what might be causing this?
