By book do you mean the diary (Leandra's Lab Notes), or Leandra's Spell? You should be able to craft the Death Knight Bane Skillbook by drag and dropping the spell over the Vial of Leandra's Blood, or the blood over the spell.
There are no requirements to have read the spell or lab notes, or to combine them with a Source Hunter, etc.
