There was a serious bug discovered recently with the latest patch on PC, which could cause performance issues late in the game (running animations for non-lead characters, slow UI response, spell effects delayed or missing [though the casting animation was fine and the cooldown started], etc). This could be triggered, or made worse, when switching map regions, and would eventually start causing a crash on those loading screens. This was fixed yesterday, and is going through testing today.
Unfortunately, since the console patches are also affected by this issue, the fix for it needs to be integrated into the patch, which will then need to be resubmitted to Microsoft and Sony. That will likely push the release into the new year.
