Another person on PS4 reported this problem, but the crash was not reproducible with the current live game version or with the upcoming patch version (no ETA yet, just early in the new year).
When you re-tried the fight, did you vary your tactics a bit, to see if that might make a difference?
Please email supportdos@larian.com, with a brief descriptions and couple saves, one immediately before killing Evelyn and one before the start of the fight. You would first need to transfer the saves to a USB flash drive to get them onto a computer: in the Saved Data in System Storage section of the Settings menu, click Copy to USB Storage Device, select the relevant save(s) and Copy. From there you can copy it to email or upload to Dropbox or Google drive, etc, and provide a link.

Same problem here. Me and my wife have killed her several times now, each time the PS4 crashes and puts us in XMB menu.
Tried different tactics: Massive AOE spells, Kill al summoned creatures first, kill Evelin first.
So we moved on with the rest of the quests, we've completed as much as we could and tried again but with no luck :(
(We've reported each error, PS4 error reporting)

Yes, we've killed her without a crash! We did every sidequest possible including Treasure maps. Changed our entire gear sets and killed her first of all the enemy's and it worked.

I'm glad it worked properly when you went back. I wonder if it was the change in characters that helped, some other quest progression or just being able to finish the fight much quicker...

I wanted to add another case of this (what sounds like) exact same bug. My wife and I have played through the Evelyn fight a couple of times now and each time we administer the killing blow to Evelyn, the game crashes. We are playing the PS4 version of the game.
This is a pretty serious, game breaking bug... we will try to go do some side quests before attempting again and report back once we have, but this is a very major issue to have in a retail release. Very unhappy users here.

I'll add a link to this topic to the support issue I mentioned above. Unfortunately, this problem has not been encountered in testing, and was not reproducible with the only save we have gotten where it happens on the player's PS4.

We tried again tonight and we did not get the crash, which was great, but several attempts before this the game crashed and sent us back to the PS4 OS. It might be worth mentioning that we have been playing with two players. I went ahead and sent the saves on to the support e-mail you mention in case they are of use.

I have the update. Game on PS4 crashed when I killed Evelyn. I sent data upon crash.

Unfortunately, this crash was not reproducible in the one save QA has where this occurred (one save was also provided at a later point in a game, when the crash did not occur), so there was no fix for this issue in the recent patch.

And I'll add my voice to the group.... wife and I were playing along, finally got to Evelyn, and boom, crashed. Tried twice, crashed twice (sent error report via the PS4 software). We're going to go to the Black Cove till the next patch comes out, hopefully it'll be fixed by then.

This problem was recently reproduced, actually, and tracked down to an issue with Evelyn's loot generation. It should be fixed in the next update, or you can also try the workaround described above of just going back after you have done other things.

Update: I had this issue but was able to 'fix' it by going to the church area, confronting Leandra, and searching the church fully prior to fighting Evelyn.
I am also having an issue with the Twin Dungeons quest. For some reason it won't let me place the amulets on the pedestals. I will try it again now that Evelyn is defeated.
Very happy to get past that little issue though :)
One other thing I noticed that might have had something to do with the crash after Evelyn. My shared inventory was broken for two of my characters on the save game I did prior to the Evelyn fight. I couldn't see the weapons or armor in the main character's inventory on those two characters for some reason. That issue has since fixed itself though.
