If a character was cursed (wearing a certain ring, for example), it may have cancelled out the blessed effect of the cloud. Different conversation options can also result in different effects (angering the goddess can result in the surface and mist being blood rather than water), but I don't recall if you can get an unblessed steam cloud without curse involved.
