Retracing back my step, I was finally able to figure out why my Blood Rose buff sometime randomly disappear.
"The One Arena"
At the end of the fight, you will get teleported out of the fight area and back to the NPCs. When this happens, EVERY BUFF YOU HAVE WILL ALSO BE REMOVED.
This includes the Blood Rose Elixir which is supposed to be permanent.
It's really a sucker punch kind of bug that removes the strongest and limited stat boost in the game.
