Regarding the Eternal Worshipper quest, you're supposed to put her soul jar into your backpack and then give it back to her.
Mother's worst nightmare should be Fara -> Jeth -> Fara, you'll see an option about her daughter having died in a Voidwoken attack and the quest will close out saying there's nothing else you can do.
The other quests remain an issue, yes.
