Kinda agreed, although from a RP perspective I think it's a sweet idea to make you pay the consequences. (They do, sometimes. E.g. if you're asked to bring the head of somebody as proof that you killed him, and Sebille eats said head, you can usually tell your client what happened. They tend to not be impressed.)
What shouldn't happen though is that quests get stuck with no way of recovery. Ancient Neverwinter Nights had a nice feature as a catch-all: They had a magic fountain that would collect quest-essential items if you dropped/sold/destroyed them or if they were no longer obtainable (e.g. if you locked off a certain area). Not very RP-y, admittedly, but it was a nice emergency feature.
