Wanted to update the thread:
I wanted to check if all those erroneous journal entries are a chain effect of the bugged Surrender your Source dialogue option and picked to fight Lucian instead. The fight against everyone is certainly not easy but I made it eventually and indeed most of the journal entries were correct this time, except for the bit of me persuading Braccus to surrender which still did not happen.
HOWEVER. I replayed the fight again as 2 of my companions died in the previous attempt (Ifan and Sebille) and wanted everyone to survive it. I made it and this time Ifan's and Sebille's entries were just as wrong as in my original post! What's more this is what happened to Ifan:
