This happens right after the big ship battle at the end of Act 1, and you get into Hall of Echoes. At the start of Act 2 the bottom deck hatch no longer works.
This occurs for both modded and non-modded games, especially since everything was working fine prior to that game event.
This is game breaking, since you don't get access to respec mirror and Tarquin/Alexander. Of all the issues that needed fixing, I am surprised something this major was left out of a solution for patch 3.0.146.559. Can't progress gameplay, started a forum account to address this issue.
Look at how long this issue has been going on, even a Larian staff addressed it:
