Appearance is locked in to most items, so you'll have to find an item that has the appearance you like first and then Right-Click > Manage them to suit the properties to your needs.
I find it is much easier to just use the items in the Weapons menu as the thumbnail gives you a better idea of their appearance. In its current state, the Generate Item menu is pretty pointless.
